1. Assumption of Risk
I understand that participation in ice hockey training, camps, clinics, private lessons, and semi-private lessons involves inherent risks, including but not limited to:
- Falls on ice
- Collisions with other players
- Contact with pucks, sticks, boards, or equipment
- Sprains, fractures, concussions, or other serious injuries
- Equipment malfunction
- Exposure to illness
I voluntarily assume all risks, both known and unknown, even if arising from negligence, related to my child’s participation.
2. Release of Liability
On behalf of myself and my child, I hereby release, waive, and discharge:
- Blueprint Hockey LLC
- Its members, managers, coaches, contractors, and staff
- Ice rink facilities and their owners/operators
from any and all claims, demands, causes of action, damages, or liability arising out of or related to participation in Company programs, including claims arising from ordinary negligence.
This release does not apply to gross negligence or willful misconduct.
3. Medical Authorization
I authorize Company staff to seek emergency medical treatment for my child if necessary. I understand that I am financially responsible for any medical treatment provided.
I confirm that my child is physically fit to participate in hockey training activities.
4. Equipment Responsibility
I acknowledge that it is my responsibility to ensure my child:
- Wears properly fitted hockey equipment
- Uses a certified helmet and required protective gear
- Brings equipment in safe and functional condition
5. Refund & Cancellation Policy
I understand that:
- Ice time is prepaid and non-refundable unless otherwise stated
- Missed sessions are not guaranteed makeup sessions
- The Company may cancel or reschedule due to weather, facility issues, or unforeseen circumstances
6. Code of Conduct
I agree that my child will:
- Follow coach instructions
- Show respect to coaches, players, and staff
- Maintain appropriate behavior at all times
The Company reserves the right to remove any participant for unsafe or disruptive behavior without refund.
7. Governing Law
This agreement shall be governed by the laws of the State of Colorado.
8. Acknowledgment & Electronic Signature
By checking the box at checkout and completing registration, I acknowledge that:
- I have read and understand this Waiver and Release of Liability
- I am the parent or legal guardian of the participant
- I voluntarily agree to its terms
- This electronic agreement is legally binding
